Registration Forms for Parents

Registrations are held to verify student enrollment and to share information with students and parents concerning class assignments, bus routes, lunch programs, school rules and regulations and other information about the upcoming school year.

If you would like to download forms, complete them and take with you to registration, please see the links below.  You may also complete these forms onsite at the school. 

Returning Students (attended KCSD schools in 2011-12):

• Health Information Form (Elementary and Middle students only)

• Transportation Information for Bus Drivers (Bus riders only)

• PowerSchool Parent Portal Access Forms (only complete these forms if you did not use Parent Portal for this student last year to access grades and attendance online): Parent Portal Acceptable Use Policy and the Custody Identification Form

Parents of returning students who have already established the student’s residency will be required to provide a current (two months or less) utility bill (electric or water) that contains the student’s address in order to register.

New Students (did not attend KCSD schools in 2011-12):

• McKinney-Vento Act Identification Form

• Health Information Form

• Early Childhood Form (4-Year-Old Kindergarten Students only)

• PowerSchool Parent Portal Acceptable Use Policy (For online grade access)

• PowerSchool Parent Portal Custody Identification Form

Kershaw County School Board policy requires parents to bring to registration all of the following documents to enroll a student for the first time: a copy of the child's birth certificate, the child’s South Carolina Certificate of Immunization, a parent/guardian’s valid photo id with correct & current address (such as a driver's license or Dept. of Transportation id), a tax document with the current and correct address, a check stub with the current and correct address and either a mortgage or lease agreement or legally notarized documentation from the landlord.  A child’s registration will not be complete without this documentation.
